Geriatric Specialist Case Manager

Capital BlueCross seeks a PA-licensed RN to coordinate case  

management services for geriatric members, with an emphasis on  

promoting quality care and cost effective outcomes.

You will use a collaborative process to assess member needs; review  

service/resource options; develop/ implement care plan; monitor/ 

document progress; and participate in development of programs within  

clinical management to meet the needs of our geriatric population.

This position requires 3 years of recent community health based  

geriatric clinical experience; PC skills (MS Word/Access); ability to  

develop/ design clinical programs targeting geriatric population; a  

PA driver’s license; and access to own vehicle for travel when  

necessary. Knowledge of URAC and CMSA Standards for case management;  

Act 68/NCQA/ERISA/ Medicare guidelines; and managed care principles/ 

emerging health treatment modalities is essential. Geriatric Nurse  

Practitioner preferred.
